Fixed angle devices, such as angled blade plates and dynamic condylar screws (DCSs), are preferred and often accompanied by autogenous bone grafting. The bone stock of the inferior femoral head has usually not been violated by the prior device. Implants used for revision internal fixation are typically selected according to the quality and location of the remaining bone stock of the proximal femur. In this regard, orthopedic surgeons should spare no effort to preserve native bone and achieve fracture union with this procedure, especially in active patients younger than 50 years of age. Unlike hip arthroplasty, which is characterized by limited longevity, revision internal fixation preserves the femoral head thus, further revision surgery due to prosthesis abrasion is unnecessary. However, open reduction and revision internal fixation with or without osteotomy or bone grafting have been reported to achieve high union rates and few complications. In this review, we discuss novel strategies regarding salvage options and surgical techniques to improve the outcome of patients with failed internal fixations of IT fractures.įull size image Femoral head salvage proceduresįailure of fixation of IT fractures in young patients is exceedingly rare. Accordingly, management of these cases has been reported with increased risks of perioperative morbidity, prolonged operative times, escalated blood loss, frequent intraoperative fracture, and a high rate of early dislocation. Several technical hurdles emerge in this situation, including residual bone deformity, distorted soft tissue anatomy, broken implants, poor bone stock, and femoral deficiency. Salvage osteosynthesis and conversion hip arthroplasty remain the mainstays of treatment for the failed internal fixation of IT fractures rather than conservative, nonoperative therapy, which is limited to incredibly infirm patients. Surgery indications include implant failure, nonunion, malunion, fracture, dislocation, femoral head necrosis, posttraumatic arthritis and infection. Failed treatment of IT fractures leads to remarkable disability and pain, which may cause complications associated with prolonged recumbency and affect the vital prognosis of these fragile patients, thereby necessitating effective surgical intervention. Although most IT fractures can be treated successfully with contemporary surgical techniques and internal fixations such as intramedullary nails and sliding hip screws, clinical failures still occasionally occur, with reported data indicating a range from 0.5 to 56% depending on the fracture type, patient status, and quality of the reduction and fixation. Intertrochanteric (IT) fractures are common, accounting for almost half of all hip fractures and resulting in a great burden on orthopedic services. Overall, the salvage of failed internal fixations of IT fractures with properly selected implants and profound techniques can lead to the formulation of valuable surgical strategies and provide patients with satisfactory clinical outcomes. Technical challenges include a difficult surgical exposure, removal of broken implants, deformity correction, critical bone defects, poor bone quality, high perioperative fracture risk, and prolonged immobilization. In physiologically young patients, care should be taken to preserve the vitality of the femoral head with salvage internal fixation however, for the elderly population, conversion arthroplasty can result in early weight bearing and ambulation and eliminates the risks of delayed fracture healing. Salvage options are determined according to patient physiological age, functional level, life expectancy, nonunion anatomical site, fracture pattern, remaining bone quality, bone stock, and hip joint competency. There is an urgent need to establish a profound strategy for the effective surgical management of these fragile patients. Failed treatment of intertrochanteric (IT) femoral fractures leads to remarkable disability and pain, and revision surgery is frequently accompanied by higher complication and reoperation rates than primary internal fixation or primary hip arthroplasty. ![]() And, that is why marking a diode on your PC board with the plus sign (+) is not good practice.ĭuane Benson is the Chief Technology Champion at Screaming Circuits. However, with a zener diode, or TVS, it's not necessarily true. For the common barrier diode, or rectifier, it's a pretty safe bet. Because it’s not always true that current flows through a diode from the anode to the cathode. For example, A “+” or “-“ sign isn’t good enough. Relying on +, - or _ are not definitive in what they indicate and are not recommended. If you are producing your board without silkscreen, you can put the mark in the copper layer, or submit a clear assembly drawing with the other board files. ![]() An "A" adjacent to the Anode on the board works too, though it’s less commonly used. “K” is used because “C” could imply that the spot wants a capacitor. You may also place a "K" for Cathode adjacent to the cathode. The OUT1OUT4 PCB pads provide connec- tions for connecting each HB LED strings cathode to. We set up our build per the vendors' specified definition of pin 1, then direct our operators to assume pin 1 is the cathode, unless otherwise noted in our work instructions. Thus, older designs have cathodes as pin 1 and newer ones have anodes. The preferred method is to place the diode schematic symbol in the silk screen. each HB LED strings anode to the DC-DC preregulator output. Our primary LED supplier changed their specs to show their pin-1 from Cathode to Anode a few years ago. ![]() To ensure the best accuracy, we recommend extra care in marking your diodes to remove any ambiguity. Guidelines for diode polarity mark silk-screening-the diode symbol, “K” for Cathode or “A” for Anode or. When you start looking at the CAD libraries, you not only have all the differences from that manufacturer, you may also have different markation schemes from each CAD package developer and from each library builder. It all depends on whether the diode is a rectifier, an LED, a uni-directional TVS, part of a daisy-chain and a host of other considerations. Barrier diodes can be either orientation. LEDs are usually cathode negative, while zeners and uni-directional TVS diodes can be cathode positive. If you use surface mount diodes or LEDs, you probably understand the challenges involved in correctly and consistently indicating diode polarity. A key element of that accuracy is our understanding of your board and the component markings. Standard diode symbol (Source: Duane Benson) What you need to do is design your board with the assumption that polarity will not be clear, and then work to make it clear. Have you ever had an LED or other diode placed backwards? PCB assemblers work hard to place every component, from the largest, highest pin-count logic chip, down to the smallest passive components and micro wafer scale BGAs, correctly every single time. For your reference, those standards (IPC-610) state that, on your PCB, a rotation of zero has the diode horizontal with the cathode on the left side. ![]() Non-holiday weekdays are typically quiet, and timing your walk, hike, or run with sunrise or sunset create will create a beautiful backdrop with the river, particularly at the suspension bridge. She was pro I was con.Ī few weeks later, our trek to DSW for back-to-school tennis shoes started out serenely enough. We had been arguing over the “appropriateness” of sky-high heels for two years already. However, given the advantages that height supposedly confers from higher pay to increased desirability as well as the long history of men in heels, the real question is why don’t the majority of men wear heels today? Indeed, the most curious thing about men in heels may be our current cultural distress over the subject. When my fashionista daughter got a $40 gift card to DSW for her 13th birthday, I knew the stage had been set for battle. For most men, however, even an extra inch of a pair of business brogues can provide highly destabilizing and call their masculinity and their judgement into question. It in fact, there are some lifestyles where heels have remained central to expressions of masculinity and are worn without raising eyebrows the rugged cowboy in heeled boots is the perfect example. Even after heels fell out of men’s fashion in the 18th century, there have been moments when they have been reintegrated into the male wardrobe – not as a ways of challenging masculinity but rather as a means of proclaiming it. When heels were first introduced into Western fashion around the turn of the 17th century, men eagerly embraced them and they continued wearing heels as expressions of power and prestige for over 130 years. Today, the thought of a man in heels is met with disbelief and conjures up notions of transgression. |
